Training Certificate Request Form

NOTE:

Every time you download a training title, it is recorded in your individual Training Log with the title, date and number of hours that particular training is equivalient to.

At any time, you can download your Training Log, and then go to the Training Certificates page.

You can print one Training Certificate for each training module you’ve completed.  (Couples:  Print one certificate in each of your names.)  These certificates are provided as a service to our members, on an “honor” basis with the understanding that the member has actually read, studied, discussed and/or completed the specific training before printing a Training Certificate.  FosterParentTraining.com is not responsible for verifying any member’s training!

Each certificate will have your name, the date, training module title, and the hours equivalency.  “Hours Equivalency” is based on the approximate number of hours the same training would take if it were offered “live”.

INSTRUCTIONS:

    1. Enter your First and Last Names as you want them on your Certificate.
    2. Download & Print your Training Log
    3. Enter the Training Date.  (Note:  Don’t date 20 hours of training with the same date!)
    4. From your Training Log, select the Training Title below for which you’d like a Training Certificate.
    5. At the bottom of the Title Table below, click on “See & Print Your Training Certificate””.
    6. To Print your Training Certificate:  Click on the “Print Page” button, or do either a “Right Click” anywhere on the page or click on “File” in the upper tool bar, then click on “Print”.
